About Complex Continuing Care Our vision is to provide an accessible and seamless network of medical and restorative services for those individuals living with or recovering from a chronic illness, across the continuum of care and in partnership with other providers. Our patients receive a defined multidisciplinary assessment and treatment with a finite length of stay.

Complex continuing care is located at the Freeport Health Centre and replaces the former chronic care program. Our services each have their own purposes, goals, and eligibility criteria. Admission is based on application, a duty fulfilled by your health care professional. The forms and fax numbers for each program are available in the service descriptions.
