Diabetes is a chronic disease and adopting a healthy life style, self-monitoring and self care on a daily basis is critical for patients to maintain their health.
The goal of the Diabetes Education Centre is to help patients understand their condition and participate in the daily management of their diabetes. The centre provides education and support and helps patients and their families work in partnership with their physician to better manage their diabetes and maintain their health.
A referral from your doctor is preferred so that the diabetes educators will have copies of your medical data to be able to provide you with the best care. Any health care provider can make a referral for an individual.
