About Our Geriatric Care Programs Grand River Hospital offers a variety of inpatient and outpatient specialized geriatric services. These programs are located at Grand River Hospital's Freeport Health Centre where we assess, provide diagnostic investigations, treatment and care planning to patients with complex geriatric and medical conditions. The goal of these programs is to improve functional independence and prevent institutionalization. INPATIENT CLINICS
The geriatric assessment unit provides comprehensive assessment and treatment for individuals with complex medical and psychogeriatric conditions. Patients are admitted for a 14 to 30 day hospital stay.
OUTPATIENT CLINICS
The geriatric outpatient clinic provides comprehensive medical assessment, education and treatment of the geriatric client.
The day hospital is an outpatient assessment and rehabilitation service, which provides inter-disciplinary care for patients with age-related illnesses. Goals of treatment are to improve quality of life and achieve an optimal level of functioning.
The psychogeriatric outpatient clinic provides comprehensive psychiatric assessment, education and treatment to geriatric clients.
