Patient Services The rehabilitation program at the Freeport Health Centre offers both inpatient and outpatient services. General rehabilitation is an inpatient therapy program. General rehabilitation aims to increase functional independence, foster quality of life, and to realize a safe and stable discharge for its clients. It provides rehabilitation care for inpatients who are stable, require and are able to tolerate therapy sessions of between one and three hours daily, over a period of ten or more days. Neurological conditions predominate, followed by pulmonary, amputations, and injury related cases. Clients are accepted from 16 years of age, although the average age of current clients is approximately 64 years of age. Referrals are accepted from throughout the Waterloo Region. Eligibility:
ADMISSION FORMS: Fax to: (519) 894-8324 For more information please contact the Resource Nurse at (519) 749-4300, ext.7335 The outpatient services offered include: The neuro rehabilitation clinic will continue to provide individual, selective intensive therapy for regional residents aged 16 years and older. Clients will attend the clinic for up to five sessions per week. They will receive one-on-one therapy in one or more disciplines, including physiotherapy, occupational therapy, and speech language pathology supported by recreation therapy and social work. For more information please call (519) 894-8340. The geriatric day hospital provides rehabilitation therapy in a group setting or to individuals, on a day attendance basis. The goal of the day hospital is to increase functional independence and quality of life as a means of avoiding or delaying admission to other forms of care. The day hospital serves a predominantly frail, elderly clientele. (The average age is currently 70 years.) Exceptions are made for younger adults who otherwise meet admission criteria. Clients have predominantly physical rehabilitation needs and require attention from at least two therapies and/or medical care. The day hospital offers physiotherapy, occupational therapy, speech language pathology, nursing, nutrition, pharmaceutical advice, recreation therapy, social work, and medical services. It has access to a geriatric specialist and to a psychogeriatrician. Other on-site diagnostic modalities available to the clients include basic radiology, audiology, and vision services. Caregiver education is also a significant element of the day hospital services. For more information please call (519) 894-8340. This is a treatment service for skin problems, which is primarily used by outpatients. A referral from a dermatologist is required. For more information please call (519) 749-4300, ext. 7040. The pulmonary rehabilitation clinic provides an interdisciplinary service tailored to the individual needs of clients with a functionally limiting lung disease. The service entails education and exercise with the goal of maintaining an individual in the community at the highest functional level allowed by their condition, and avoiding hospital stays. Clients will attend an eight week session of care. Additional elements of the clinic include a drop-in and exercise program, and support group. Client age ranges from 21 to more than 80 years old. Prosthetic and Orthotic Clinic The prosthetic and orthotic clinic provides consultation, assessment, treatment, and follow-up services to amputation clients or clients requiring an orthotic device. Admission is open to clients of any age, who are potential candidates for prosthesis and require assessment for fit and function; and, any person with a disability who needs an assessment of fit and function. The clinic interdisciplinary team will coordinate with community resources to optimize quality of life for clients, under the medical direction of a consultant physiatrist. Freeport Augmentative Communication and Technology Services (FACTS) The FACTS clinic provides a specialist assessment and consultative service to residents of South West Ontario with acquired or congenital conditions, which prevent speech or written communication. The FACTS interdisciplinary team will aim to maximize the communication potential of clients through the application of communication methods and systems including technical devices.
