Patients should come prepared before they arrive at Grand River Hospital.
A suggested list of items to bring include:
- Your completed pre-admission form.
- Photo ID, such as a driver's license.
- Your Ontario Health Insurance card.
- Your Grand River Hospital card, if applicable.
- Private health insurance information, if applicable (*it is the patient's responsibility to know what their extended health covers).
- Cheque, credit card, or debit card to cover use of hospital services, such as telephone fees.
- Pajamas, nightgown, bathrobe, and slippers, if you wish.
- Personal toiletries, such as shampoo, toothbrush, toothpaste, denture cream, and face creams.
- Comfortable clothes to wear home, such as a track suit.
- Small amount of money to purchase newspapers, magazines, etc.
- Assistive devices, such as canes, walkers, crutches, hearing aid devices, or magnifying glasses, labeled clearly with your name.
- A list of prescription medications, dosages, and schedule. (Please ask your physician beforehand if you should bring any of your medications to the hospital).
- Names and phone numbers of family members or friends to contact in an emergency.
- Written list of past hospitalizations, illnesses, surgeries, and allergies.
Women who are having a baby will need to bring additional items. Please see the childbirth services section of this website for more information.
Please do not bring:
- Valuables such as large amounts of cash or jewelry.
- Personal electrical appliances, such as hair dryers, curling irons, electric shavers, heating pads, television sets, radios, and cellular telephones. These devices can interfere with hospital medical equipment.
